What comes back after you send an 837
When a professional claim leaves your system as an EDI 837, it does not go straight to the payer's adjudication system. It passes through checks first, and each check answers with its own file. Knowing which file says what is the difference between fixing a rejection in ten minutes and discovering it a month later, when the payment never arrives.
| File | What it checks | What a rejection means |
|---|---|---|
| 999 | The structure of the file: X12 syntax, segments, required elements. | The transaction set was not processed and never reached the payer. |
| 277CA | Each claim against business rules: identifiers, codes, dates, payer edits. | That claim, and only that claim, was not entered into adjudication. |
| 835 | Nothing to fix: it is the remittance, what the payer paid or denied. | A denial on an 835 is an adjudicated claim, not a rejection. |
CMS describes the split simply: the TA1 and 999 reflect technical problems, while the 277CA reflects a data problem that has to be handled by the billing team. On the Medicare side, the front end returns a 277CA for each accepted or rejected claim, so one bad claim no longer sends back a whole batch.
Through Availity, the order is predictable. A 999 comes first; a rejected transaction set goes no further and is not routed to the payer. When the 999 accepts the file, Availity returns its own claim-level report, usually within minutes and up to 24 hours. The payer's own response follows, typically within 24 to 48 hours, and some payers answer later still.
How to read the STC segment
The answer for each claim sits in the STC segment of the 277CA. Its first element is a composite of three parts: the claim status category code, the claim status code, and, when needed, the entity the problem is about (the billing provider, the subscriber, the payer).
- STC01-1, the category code: the verdict (received, accepted, rejected).
- STC01-2, the status code: the reason, as a number from the X12 code list.
- STC01-3, the entity code: who the reason is about, for example 85 for the billing provider.
- STC03, the action code: WQ for accept, U for reject.
- STC12, the free-text message: the payer's explanation in words.
A message does not mean a rejection. Availity can add an informational warning to a claim that was accepted, and X12 has no separate accepted with warning status in the 277CA. Only the category code and the action code decide whether the claim went in or came back.
Zeumatic builds these claims from your authorizations, sends them through Availity and matches every payment back. Try it with your own data.
Start free trialCategory codes: A means acknowledgement, not accepted
The most common misreading of a 277CA is to treat every A code as good news. In the X12 list, the A family means acknowledgement. Some acknowledgements are acceptances; others are rejections.
| Code | Official meaning (X12) | What to do |
|---|---|---|
| A1 | Received. This does not mean the claim has been accepted for adjudication. | Wait for the next response. |
| A2 | Accepted into the adjudication system. | Nothing to fix; watch for the 835. |
| A3 | Returned as unprocessable: rejected, not entered into adjudication. | Correct and resubmit. |
| A6 | Rejected for missing information. | Add what the status code names, resubmit. |
| A7 | Rejected for invalid information. | Fix the value the status code names, resubmit. |
| A8 | Rejected for a relational field in error. | Fix the combination of fields, resubmit. |
X12 itself says the codes that indicate a claim or service data issue, A3, A6, A7 and A8, all mean rejected. The pending P codes belong to the separate claim status inquiry transaction and are not used in a 277CA's claim and service loops.
The status codes behind most rejections
The category tells you whether to act; the status code tells you what to change. These are among the codes care providers meet most often, in X12's own words.
| Code | X12 wording | Where to look |
|---|---|---|
| 21 | Missing or invalid information. | Always paired with another code that names the field. |
| 33 | Subscriber and subscriber id not found. | The member ID on the claim, against the plan's card or authorization. |
| 30 | Subscriber and subscriber id mismatched. | Name, date of birth and member ID must match the plan's records. |
| 97 | Patient eligibility not found with entity. | Coverage on the date of service, and the right plan. |
| 116 | Claim submitted to incorrect payer. | The payer ID: the member may belong to another plan. |
| 164 | Entity's contract/member number. | The member or contract number of the entity named. |
| 187 | Date(s) of service. | Dates outside the authorization or in the future. |
| 562 | Entity's National Provider Identifier (NPI). | The billing or rendering NPI and its enrollment with the plan. |
| 145 | Entity's specialty/taxonomy code. | The taxonomy the plan has on file for your NPI. |
| 496 | Submitter not approved for electronic claim submissions on behalf of this entity. | Your EDI enrollment with that payer. |
| 54 | Duplicate of a previously processed claim/line. | Whether the day was already billed. |
How to fix and resubmit a rejected claim
A rejected claim was never received by the payer in the sense that matters. CMS says rejected or returned transactions are not classified as received, and that a resubmitted claim that was previously rejected counts as an initial claim. In practice, a corrected rejection goes back as a new original claim, not as a replacement of a claim the payer never accepted.
- Read the category code first: only A3, A6, A7 and A8 call for a correction.
- Read the status code and the entity code to find the field, then the STC12 message for the detail.
- Correct the source of the error in your records, not just the claim, so the next batch does not repeat it.
- Rebatch only the rejected claims with a new interchange control number. Availity's guide says you do not need to include claims that were already accepted.
- Keep the rejection on file. Timely filing is counted from dates of service, so a rejection found late can cost the claim.
Frequency code 7, replacement of a prior claim, and 8, void, are for claims the payer already holds. The official CMS-1500 instructions say this item is not intended for original claim submissions, which is what a corrected rejection is.
How Zeumatic handles 999 and 277CA files
Zeumatic sends claims through Availity and reads every 999 and 277CA that comes back. Each response is matched to the claim it answers; the verdict comes from the category code, never from the presence of a message; and the payer's reason, including the STC12 text, is shown on the claim and gathered on a Payer rejections page.
Because one claim is built per served day, a rejection points at one day and one reason, and the daily checks list what blocks billing before the next batch leaves: an expired authorization, a missing plan code, a member ID that does not match.
Questions
Is a 277CA rejection the same as a denial?
No. A rejection means the claim never entered the payer's adjudication system; you correct it and send it again. A denial is an adjudicated decision that arrives on the 835 remittance and follows the appeal or corrected claim rules of the plan.
My 277CA says A1. Was the claim accepted?
Not yet. X12 defines A1 as received, and says it does not mean the claim has been accepted for adjudication. Wait for an A2, or for a rejection code.
Should I use frequency code 7 to resend a rejected claim?
Generally no. A rejected claim was never accepted, and CMS treats a resubmitted rejected claim as an initial claim. Frequency 7 replaces a claim the payer already has. Check your plan's manual for any exception.
Why does a claim with a message still show as accepted?
Availity can attach an informational warning to an accepted claim. Only the category code and the STC03 action code decide; a message alone is not a rejection.
