Florida's long-term care runs on managed care plans
In Florida, most home and community-based services reach Medicaid members through managed care. The Statewide Medicaid Managed Care program (SMMC) enrolls eligible older adults and adults with disabilities in a Long-Term Care plan, and that plan authorizes the services a provider delivers: meals at home, personal care, homemaker and respite visits, adult day care, assisted living. For the provider, every one of those services is a claim to a specific plan, under a specific authorization.
Zeumatic was built here, with Florida providers, around that reality. Each person you serve is tied to their plan and to the plan's authorization; AI reads the authorization when you upload it; and claims are sent to the plan through Availity, the clearinghouse Florida's plans receive claims from. Acknowledgments and remittances come back the same way and are reconciled for you.

Each person you serve carries their Medicaid ID, their LTC or PACE plan and the plan's payer code, picked from a list rather than typed.

Upload the authorization the plan sent you. AI fills in the service, the units and the dates; your team confirms, and Zeumatic watches the end date.

Send the claim through Availity
Claims are built per served day under the authorized service (S5170 for meals, T1019 for personal care, S5100 for adult day care, T2031 for assisted living) and sent through Availity.

Florida's managed long-term care program. LTC plans authorize home-delivered meals, personal care, homemaker, respite, adult day care and assisted living services, each billed to the member's plan.
PACE
The Program of All-Inclusive Care for the Elderly covers frail older adults through a PACE organization, which authorizes and pays the services its participants receive, meals among them.
Private pay
Families and facilities that pay you directly are invoiced from the same records as your plan-covered members.
